To file a claim for the $15 million settlement, customers can visit the settlement website cashappsecuritysettlement.com and submit a claim form. The deadline for filing a claim is November 18.

According to the settlement website, customers who were impacted by the data and security breaches may submit claims for up to $2,500 for reimbursement of out-of-pocket losses, including costs for credit monitoring or identity theft insurance, requesting a credit report or a credit freeze, and overdraft fees that haven't been refunded.

The settlement also allows customers to claim for up to three hours of lost time, at a rate of $25 per hour, and reimbursement for transaction losses. If there's not enough money in the settlement to pay every approved claim in full, payments will be made on a reduced pro rata basis.
